From the 2nd point regarding distance, the ball came off a Barcelona player first in close proximity, before hitting the AC Milan player's hand. This gave him no time to react/ move out of the way, therefore not being deliberate.

I referee quite frequently and one of the key things they stress at clinics is whether or not the hand is in a natural position.
